Clinico-pathological Analysis of The Acute Appendicitis in Korean Soldiers

Abstract
Acute appendicitis is one of the most common surgical disease in army medical field. The cases are chiefly composed of adolescent male in army.
The authors performed a retrospective analysis on one-hundred seventy five patients who received appendectomy under the diagnosis of acute appendicitis at Department of General Surgery, Dae-Jeon Armed forces General Hospital, from May 1987 to december 1988.
The results obtained were as follows;
1. Age distribution is between 16 years and 44 years, and the mean is 22.1 years(SD ¡¾3.7 years), and they are all males.
2. On histopathological classification, acute appendicitis was 19 cases(10.9%), acute suppuratve appendicitis was 50 cases(28.6%), acute gangrenous appendicitis was 38 cases(21.7%), perf- orated appendicitis was 29 cases (16.5%), and non-inflammed appendix was 39 cases (22.3%).
3. The most common clinical symptom was right lower quadrant abdominal pain (67.4% ).
4. The mean of peripheral blood leukocyte count were as follows; non-inflammed appendix, 1 0,100/mm3i(¡¾3,000), acute appendicitis, 14,800/mm3(¡¾5,300), acute suppurative appendicitis, 13,400 / mm3 ( ¡¾4,300 ), acute gangrenous appendicitis, 16;000 / mm3 ( ¡¾3,700), perforated appendicitis, 16,900 / mm3 (¡¾ 5,500 )
5. Sixty patients (55.3%) out of one-hundred seventy patients visited with in 24 hours from or set
of symptoms, and fifty-nine patient (46.9%) out of one-hundred seventy five patient were
received an operation within 24 hours from onset of symptoms.
6. The mean length of the vermiform appendix was 7.lcm(SD ¡¾ 1.7cm). 7. Most common location was retrocecal position (43.4%). 8. The ABO types of non-perforating appendicitis were as follows ; A(38.1%), B(27.4% , 0(2 9.8%6), AB (4.7% ), and that of perforating appendicitis were as follows; A(41.7?/o), B (20 8%u ), 0(29.2%), AB(8.3%).
9. The incidence of wound infection was 7.4%.
